Instructions:

MMDC forms are currently available in two formats and use the following icons for identification:

  1.  Adobe® PDF, you will need the Adobe® Acrobat® Reader® (which is available for free via download from the Adobe website) to be installed on your computer in order to view or print MMDC forms bearing this icon. Forms in this format are typically intended to be printed out locally, saved for your reference or filled out by hand or typewriter and faxed or mailed to MMDC.  

    When downloading forms for later use, please note the version number on the bottom of the form, compare it to the version number on this webpage to ensure you are using the latest version.

  2.  Microsoft® Word®, you will need Microsoft® Word® to be installed on your local computer in order to edit and submit forms bearing this icon to MMDC. Forms in this format are typically intended to be downloaded as a file and edited on your local word processor and submitted as a file attachment via e-mail to MMDC. Additionally these forms can be printed and saved for your records using your own word processor.

Please note: MMDC provides forms in these formats for convenience only and does not endorse, warranty or require the use of these products.  All MMDC forms are available via fax and U.S. Mail upon request.

Fee Schedule

Fees (to be paid in separate checks)
  1. Administrative Fee: $150 per party, payable to NCCPAJ/MMDC.
    The administrative fee relates to the first two sessions of the ADR process.
    Additional sessions may result in additional administrative fees.

  2. Mediator/Arbitrator Fee: $150 per hour, payable to selected neutral.
    The neutral fee relates to ADR processes involving up to 3 parties.
    Additional parties may result in additional neutral fees.

Billing

Each party will be invoiced for fees in advance for the initial session. Any additional time will be invoiced at the end of each session. Payments must be received in advance of the initial session or the session will not go forward.

Cancellation/Postponement Policy

Payment must be received in advance of the initial session or it will not go forward. Cancellations or postponements must be received at least three (3) business days prior to the session or a late penalty equivalent to the administrative fees will be imposed on the party initiating the change. The canceling party is responsible for notifying the other parties and the MMDC of the change in schedule. A late-arriving party will be charged full fees for the delay period if the session goes forward.

A no-show party will be responsible for the payment of all parties’ fees as though the session had occurred. If no parties attend the scheduled session, they will be held equally responsible for payment of fees as though the session had occurred.
